THE GOODS AND SERVICES SHOWN BELOW ARE THOSE WE CAN PROVIDE TO OUR CUSTOMERS.  YOU MAY CHOOSE ONLY THE ITEMS YOU DESIRE, HOWEVER, ANY FUNERAL ARRANGEMENTS YOU SELECT WILL INCLUDE THE MINIMUM CHARGE FOR OUR SERVICES.  IF LEGAL OF OTHER REQUIREMENTS MEAN YOU MUST BUY ANY ITEMS YOU DID NOT SPECIFICALLY ASK FOR.  WE WILL EXPLAIN THE REASON IN WRITING ON THE STATEMENT WE PROVIDE DESCRIBING THE FUNERAL GOODS AND SERVICES YOU SELECT.  PRICES SUBJECT TO CHANGE WITHOUT NOTICE.

PROFESSIONAL SERVICES:

 Transfer of remains to funeral home - (50 mile radius) $195
* Embalming - except in special cases, embalming is not required by law.  embalming may be necessary, however, if you select certain funeral arrangements $400
* Other Preparation of Body - this includes, but is not limited to, restoration when necessary, special cosmetology, hairdressing, special care of autopsied remains, washing and disinfecting of the remains, dressing of remains $250
* Minimum Services of Staff - securing and recording vital statistics and permits, arrangements with clergy, cemetery, crematory, and other consultation with family.  This fee is for our services, and will be added to the total of  funeral arrangements you select.  This fee is already included in our charges for direct cremations, immediate burial and forward/receiving remains. $845
* Additional Services of Funeral Director and Staff - arranging and supervision of ceremony/visitation, 24 hour attention to details, death notices and obituaries, handling and care of floral pieces, assistance with forms. $495

FACILITIES:
* Use of Facilities for Funeral Ceremony - or use of equipment and staff for church service $350
* Use of Facilities for Visitation - 1 day or portion thereof $250
* Basic Facilities - preparation/holding room, arrangement offices, administration areas, reception area $180

MOTOR EQUIPMENT:
* Hearse - use of hearse for funeral service $125
* Mileage - mileage per mile outside of 50 mile radius $2/mile

DIRECT CREMATION:

 Direct Cremation - no viewing, no ceremony, includes transfer of remains to the funeral home, preparation of the body for cremation, basic services of the funeral director and staff, 24 hour attention to details, crematory charges. $1395
